CALIFORNIA ALL-PURPOSE ACKNOWLEDGEMENT
The act of acknowledging something or someone; from the legal point of view it's a declaration of a fact to give it legal validity. Both acknowledgment and acknowledgement appear throughout the English-speaking world, but acknowledgment, without the middle e, is preferred in the US.

The purpose of an acknowledgment is for a signer, whose identity has been verified, to declare to a Notary or notarial officer that he or she willingly signed the document.
A California notary acknowledgment form is filled out by a notary public after certifying the authenticity of the signatures. Sometimes the document has the notarial act, certificate in the document; sometimes it's not so the notary will attach an Acknowledgment to the document signed.
Chapter 197, Statutes of 2014 amends California Civil Code section 1189 to require the addition of the following specific disclaimer to the top of the form certificate of acknowledgment: "A notary public or other officer completing this certificate verifies only the identity of the individual who signed the document to which this certificate is attached, and not the truthfulness, accuracy, or validity of that document." The disclaimer must be legible and in an enclosed box. The certificate of acknowledgment (PDF) must be in the form set forth in California Civil Code section 1189.
